How do I collect and return my sample?
Use one swab per cheek for 60 seconds, complete and sign the informed consent and requisition form, allow the samples to dry, and return them in the provided mailer. Follow the instructions included with your kit.
How much does the AgeWell DNA program cost?
The total program price is $250: $50 at enrollment followed by four additional monthly payments of $50. The program includes the collection kit, laboratory processing and personalized printed nutrition report. The separate $300 health-voucher benefit has its own redemption terms and requirements.
